Important note regarding the SMETA Audit

This form contains questions for SMETA audit request and will help us to prepare a binding offer. Please ensure that the information in this form is accurate and complete. During the audit, if it is identified that the site scope/information provided differs from what was confirmed during offering phase, and this is due to a lack of transparency it may be ended with the abortion of the audit or NC (Non-Compliance or Non-Conformity).

Please give your data based on whether the audit scope will be a multi-client audit (assessing workers deployed across a range of client organizations) or a single-client audit (where the audit is commissioned by a specific client) and the scope is narrowed to only include workers related to that service contract. Please consider that the audite that restricting the scope to a single client may mean that other your clients may not accept the audit. Also, the audit scope shall be limited to operations within a single country unless pre-approved by the audit requestor and Sedex.
